The Anti-Thyroid Peroxidase Antibody (Anti-TPO), Serum test detects antibodies directed against thyroid peroxidase (TPO), an enzyme essential for the production of thyroid hormones. In autoimmune thyroid diseases, the immune system mistakenly attacks thyroid tissue, leading to inflammation and impaired thyroid function. This test is commonly used to diagnose Hashimoto's thyroiditis, the most frequent cause of hypothyroidism, and is also helpful in evaluating Graves' disease, an autoimmune cause of hyperthyroidism. Anti-TPO antibodies may be detected even before symptoms or abnormal thyroid hormone levels develop, making the test valuable for early diagnosis and risk assessment. The Anti-TPO test is often ordered along with TSH, Free T4, Free T3, and sometimes Thyroglobulin Antibody (Anti-Tg) to provide a comprehensive assessment of thyroid health. It may also be used in pregnant women at risk of thyroid dysfunction and to monitor autoimmune thyroid disease over time.
